Job Description

Hospice Registered Nurse (RN) - Available locations: Atlanta GA. Seasons Care Hospice is seeking compassionate and experienced RNs to join our team as Hospice Case Managers serving the Ellijay GA Full-time positions with flexible scheduling.
Position Overview:
Our Hospice RN Case Managers plan, coordinate, and deliver patient-centered hospice care. This role includes assessing patient needs, developing care plans, implementing interventions, and collaborating with families and the interdisciplinary team to ensure comfort and quality of life.
Schedule:
Full-time Weekend availability required (based on patient needs) Every other weekend (based on patient needs)
Qualifications:
Active RN license Hospice experience preferred Strong clinical, organizational, and communication skills Compassionate, patient-focused approach to care Able to commute to diffrent locations. Join a team committed to providing the best care possible for patients and their families.

$50.00 - $52.00 per hour
