We are seeking a compassionate RN to join our Admission Team!

Admission Nurse Status:
Full Time The Admissions Registered Nurse is responsible for providing information on hospice services to potential patients and families, admitting patients to Brightmoor Hospice with completion of all admission paperwork on a timely basis. Some key duties and responsibilities are: 1. Communicates to Physician the needs of the patient and family upon admission to assure comprehensive care. 2. Attends and presents patient admissions to IDG. 3. Makes initial assessment of the patient and family upon admission and reviews the hospice benefit. 4. Develops a written initial Plan of Care for each admitted patient in conjunction with the IDG, patient, family and attending physician. 5. Reports patient's admission date and findings to attending physician, and obtains verbal orders as needed. 6. Completes Informed consent/Election of benefits forms accurately and timely. 7. Reviews services available with patient and family, including on-call services and volunteer services. 8. Coordinates admissions visit with Director of Nursing or other team member(s) as needed. 9. Communicates HHA/CNA needs to RN Case Manager 10. May re-evaluate patients at time of 90-2 recertification and report to Medical Director for continued eligibility. 11. May attend and present patients at IDT for continued eligibility. 12. Documents professionally, accurately, and timely 13. Participates in on-call duties as needed. 14. Reports any potential misconduct or violation of federal or state laws and regulations to management. 15. Has a responsibility to participate in staff training and education programs. 16. Performs other duties as assigned.
