This position requires travel to client homes throughout Georgia and South Carolina to provide skilled nursing services, supervisory visits, assessments, and client education. We are looking for an RN who enjoys building relationships with clients and families while working independently in a home and community-based setting. Position Summary The Registered Nurse is responsible for providing skilled nursing services, conducting assessments and supervisory visits, educating clients and caregivers, and collaborating with physicians, case managers, and agency staff to ensure quality care and regulatory compliance. Responsibilities Perform initial and ongoing nursing assessments. Complete supervisory visits and client evaluations. Develop and update care plans. Provide client and caregiver education. Coordinate care with physicians, case managers, and other healthcare professionals. Maintain accurate and timely documentation. Participate in quality improvement activities. Ensure compliance with state regulations and agency policies. Travel to client homes throughout assigned service areas. Qualifications Current unrestricted Registered Nurse license in Georgia and South Carolina . Graduate of an accredited nursing program. Minimum of one year of nursing experience preferred. Home care, case management, or community-based nursing experience preferred. Current CPR certification. Valid driver's license and reliable transportation. Ability to travel throughout assigned territories. Excellent communication and organizational skills. Ability to work independently and manage multiple responsibilities.
