Full-Time We Are Looking for a Nurse with a Hospice Heart Peaceful Purpose Hospice is seeking a compassionate and dedicated Registered Nurse Case Manager (RNCM) to join our growing team. We are looking for more than a nurse—we are looking for someone with a true hospice heart. Hospice nursing is a calling. While clinical knowledge and assessment skills are essential, we believe the most successful hospice nurses possess compassion, integrity, patience, and the ability to support patients and families through some of life's most challenging moments. We are seeking an RN who understands the importance of dignity, comfort, advocacy, and building meaningful relationships with patients and families. Someone who leads with empathy, communicates with honesty, and provides care with excellence. Position Summary The Registered Nurse Case Manager is responsible for coordinating and managing the care of an assigned caseload of hospice patients. The RNCM serves as the primary clinical contact for patients and families, providing comprehensive assessments, symptom management, education, and coordination of care with the interdisciplinary team. Essential Responsibilities Complete comprehensive admission, recertification, and routine nursing assessments. Develop, implement, and revise individualized plans of care. Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, and other members of the interdisciplinary team. Provide education to patients, caregivers, and families regarding disease progression, symptom management, medications, and hospice philosophy. Manage pain and symptoms utilizing hospice best practices. Ensure timely and accurate clinical documentation. Participate in IDT meetings and contribute to patient care planning. Provide emotional support and guidance to patients and families. Participate in on-call rotation as assigned. Advocate for patient goals, comfort, and quality of life. What We Are Looking For The ideal candidate: Has a genuine passion for hospice and end-of-life care. Demonstrates compassion, empathy, and emotional intelligence. Operates with a high level of personal and professional integrity. Values patient dignity and family-centered care. Has strong assessment and critical-thinking skills. Is organized and able to effectively manage a caseload. Communicates professionally and compassionately. Works independently while collaborating effectively with the interdisciplinary team. Understands that hospice is about caring for the whole patient—physically, emotionally, and spiritually. Qualifications Current Georgia Registered Nurse (RN) license in good standing. required. Hospice, home health, geriatrics, oncology, palliative care, or long-term care experience preferred. Valid driver's license, reliable transportation, and automobile insurance. Ability to travel throughout the service area. Strong computer and documentation skills. Hospice Experience Preferred, Not Required Hospice experience is preferred but not required. We are willing to train the right nurse who possesses strong clinical judgment, excellent assessment skills, compassion, integrity, and a sincere desire to care for patients and families facing end-of-life challenges. $75,000.00 - $90,000.00 per year
