Position Summary:
The RN Case Manager coordinates and manages nursing care for individuals with Intellectual and Developmental Disabilities (IDD) in home and community settings. This role ensures comprehensive, person-centered care through assessment, individualized care planning, interdisciplinary collaboration, and quality oversight. The RN Case Manager serves as the primary point of contact for individuals and their families, promoting health, safety, and independence.
Key Responsibilities:

Care Coordination and Case Management:
Assess the physical, emotional, and developmental needs of individuals with IDD in home settings. Develop individualized care plans in collaboration with Supports Coordinators, physicians, therapists, and other providers. Serve as primary contact for individuals and families, ensuring communication and service coordination. Monitor progress and adjust care plans based on individual needs and responses. Schedule and coordinate care to meet individual requirements. 2.
Referrals, Staffing Coordination, and Onboarding:
Generate referrals from community partners to expand service reach. Assess staffing needs with RN Nursing Manager and Nursing Business Operations Manager to ensure appropriate skill coverage. Oversee referral processes for timely, accurate, and aligned care delivery. 3.
Collaboration and Communication:
Coordinate with interdisciplinary care teams, including physicians, therapists, social workers, and caregivers. Educate families on health management, care requirements, and home-based strategies. Develop and implement solutions for barriers to effective care, ensuring satisfaction and quality outcomes. 4.
Compliance and Documentation:
Monitor field-based RNs and LPNs to ensure accurate and timely submission of nursing notes. Provide training on care delivery best practices, person-centered planning, and internal policies. Maintain accurate documentation, including ISPs, assessments, nursing notes, and incident reports. Ensure proper incident reporting according to regulatory requirements. 5.
Health Education and Prevention:
Educate individuals and families on managing health conditions, medications, and preventive measures. Promote healthy lifestyle choices, symptom management, and quality-of-life strategies. Support independence and self-management through education and guidance. 6.
Quality Assurance and Safety:
Ensure nursing care aligns with best practices, safety protocols, and quality standards. Participate in case reviews and quality improvement initiatives. Advocate for enhancements in care quality and service delivery.
Qualifications:
Education:
Bachelor's Degree in Nursing (BSN or ADN) from an accredited program.
Licensure:
Valid RN license in Georgia.
Experience:
Minimum 1 year of nursing experience, preferably in home health or IDD care. Experience in case management or interdisciplinary collaboration preferred.
Competency Requirements:
Strong clinical assessment and individualized care planning skills. Knowledge of IDD-related health conditions and interventions. Effective communication with individuals, families, and interdisciplinary teams. Ability to work independently, manage caseloads, and prioritize tasks. Cultural competence and sensitivity to diverse backgrounds. Analytical problem-solving and decision-making skills.
Clearance Requirements:
Criminal Background Check Motor Vehicle Record Check Child Abuse Clearance Authorization to work in the
United States Work Environment:
Office-based 08:30am-05:00pm Monday-Friday. Requires travel to individual homes, community settings, and healthcare facilities.
