Job Description

Overview Join our dynamic healthcare team as a Registered Nurse Case Manager, where you will play a vital role in coordinating patient care across diverse clinical settings. This energetic position offers the opportunity to utilize your extensive nursing expertise to improve patient outcomes, streamline discharge planning, and optimize utilization management. You will work closely with multidisciplinary teams to ensure comprehensive, patient-centered care that adheres to all regulatory standards and promotes healing and recovery. Duties Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations, to determine appropriate care plans. Collaborate with physicians, specialists, and healthcare providers in hospital, outpatient, and post-acute settings to develop and implement individualized care strategies. Manage utilization review processes by evaluating diagnostic evaluations, medical records, and treatment plans to ensure appropriate resource use. Coordinate discharge planning and post-acute care arrangements, ensuring seamless transitions for patients across inpatient and outpatient environments. Oversee intake procedures for new patients, ensuring accurate documentation of medical terminology and compliance with HIPAA regulations. Participate in case management activities that include patient service optimization, behavioral health support, infectious disease care, and geriatrics. Maintain detailed documentation of patient care activities within electronic medical records while adhering to confidentiality standards. Skills Proven experience in critical care environments such as ICU, emergency room, or Level I/II trauma centers. Strong background in hospital medicine, medical-surgical units, or outpatient clinics with a focus on acute care settings. Knowledge of physiology, diagnostic evaluation techniques, and inpatient/outpatient treatment protocols. Familiarity with managed care principles, utilization management processes, and discharge planning procedures. Excellent patient assessment skills combined with the ability to interpret complex medical terminology accurately. Ability to navigate hospital experience requirements including PICU (Pediatric Intensive Care Unit), OR (Operating Room), and behavioral health settings. Effective communication skills for collaborating with multidisciplinary teams and engaging in patient service initiatives.
