Job Description
Hospice RN Case Manager Stone Mountain / Clarkston / Tucker / Chamblee / Lilburn, Georgia We re hiring a Hospice RN Case Manager who wants autonomy, meaningful patient relationships, and the opportunity to provide compassionate end-of-life care. You ll step into a clinical role focused on providing hospice care in patients homes throughout Stone Mountain, Clarkston, Tucker, Chamblee, Lilburn, and the surrounding communities. You ll collaborate closely with an experienced interdisciplinary team while maintaining independence in managing your caseload. Why this role is compelling

- Hospice experience is preferred but not required
- Home Health, ICU, CCU, Oncology, and Med Surg backgrounds are welcomed

- Base salary: $88,000 $93,000
