Registered Nurse (RN) - Valdosta, GA Unique Pediatric Hearts is currently seeking a compassionate and reliable Registered Nurse (RN) in the Valdosta, GA area to complete an initial in-home client assessment. We are a GAPP agency , and all hours of care are approved through GAPP.

Position Details:

We are looking for an RN to complete an initial in-home assessment for a new client. This visit includes completing required documentation and clinical assessment forms.

Compensation:

$100 for completing the initial in-home assessment visit

Location:

Valdosta, Georgia (in-home visit required)

Assignment:

If you are interested in working with the client after services begin, you may continue as the assigned nurse for that client (pending approval and assignment)

Important Information:

At this time, hours for this client have not yet been determined Hours will be established after the initial assessment is completed and submitted Once documentation is reviewed, it will be submitted to GAPP for final determination of authorized hours All future scheduling and service hours are dependent on GAPP approval: The client's mother is Spanish-speaking only The family does have access to a translator to assist with communication during visits and care coordination

Ideal Candidate:
