Key Qualifications:
Active RN license Home health experience strongly preferred Strong communication and collaboration skills Ability to adapt to new environments and embrace diverse clinical practices
Core Responsibilities:
Collaborate in the development, implementation, and revision of the case management program Act as a liaison among patients, families, physicians, and multidisciplinary teams to optimize care outcomes Lead coordination of patient care, ensuring timely and appropriate interventions Monitor adherence to interdisciplinary care plans and covered services, ensuring continuity throughout the patient's clinical course Consult with physicians regarding care utilization, treatment plans, and discharge processes Focus on supporting high-needs populations—including those with chronic pain and substance use disorders Coordinate transfers to tertiary centers, facilitating seamless transitions and effective information sharing Collect data for regulatory and accreditation compliance Conduct reviews on frequent emergency department patients to enhance care quality
