Job Description:
Provide skilled nursing visits in patients' homes. Complete assessments, care plans, and patient education. Perform wound care, medication management, and coordination of care. Communicate with physicians and the interdisciplinary team. Document accurately and on time in a Medicare home health environment. Reliable transportation to serve clients across the island.
