Role Responsibilities:

Collaborate with physicians, patients, and family members to develop and implement effective case management plans. Serve as a key liaison between multidisciplinary team members and healthcare beneficiaries to ensure optimal patient outcomes. Lead and coordinate patient care, monitoring the timeliness and appropriateness of services. Oversee and revise interdisciplinary care plans to reflect ongoing clinical needs and covered services. Consult with healthcare providers on resource utilization, treatment planning, and discharge arrangements. Focus on complex cases—including frequent emergency department users, chronic pain patients, and individuals experiencing homelessness—for targeted interventions. Partner with acute care case managers to ensure smooth transitions and continuity of care. Organize patient transfers to tertiary centers, ensuring complete transmission of essential medical information. Collect and analyze compliance data for regulatory and accreditation needs. Conduct focused reviews of frequent emergency visitors to improve care strategies.

Qualifications:

Active RN license. Home Health experience strongly preferred. Strong communication and collaboration skills. Ability to adapt to varying patient needs and fast-changing environments.
