Desired Qualifications:
Active RN license valid in Hawaii or compact state eligibility At least one year of recent Home Health nursing experience Proficient in case management and interdisciplinary communication Strong organizational and critical thinking skills
Key Responsibilities:
Collaborate in developing, implementing, revising, and reporting on the case management program Act as liaison among patients, families, nurses, physicians, and the interdisciplinary team Serve as a consultant on complex patient management and care coordination issues Ensure care plans are tailored to each individual's unique needs, adjusting them as care evolves Monitor diagnostic tests, treatments, and procedures for appropriateness Communicate regularly with physicians to assess resource utilization and create comprehensive treatment and discharge plans Focus on high-acuity and complex cases, including patients with chronic conditions, frequent ED visits, and social challenges Facilitate smooth patient transfers and continuity of care, including necessary data sharing Gather and submit data for compliance with regulatory/accreditation requirements Conduct focus studies to proactively manage frequent emergency department visitors
