Job Description

Overview Join our team as a Hospice Clinical Liaison (Field Sales) We are seeking a dedicated Clinical Liaison who is committed to serving patients with compassion, integrity, and professionalism. In this role, you will be responsible for coordinating and generating hospice referrals within both existing and new accounts, identifying and developing new market opportunities, and maintaining strong, ongoing relationships with hospitals, physicians, and other referral sources. As a Clinical Liaison, you will serve as the vital link between our agency and the medical community. You will educate healthcare professionals, referral partners, and the community about hospice services, while ensuring a smooth transition of care for referred patients. This position requires daily interaction with patients, families, medical professionals, and community partners to ensure continuity of care, clear communication, and accurate documentation. You will collaborate closely with agency staff to ensure patient care is delivered efficiently and effectively. Because relationship-building and community engagement are essential to this role, the Clinical Liaison regularly works in the field, meeting with referral sources and partners throughout the service area. Education :
Graduate of an accredited school of nursing.
Licensure:
Registered Nurse or Respitory Therapist currently licensed in the state. Must successfully perform and comply with all credentialing as required by hospital/healthcare facility.
Experience :
2 years experience in a clinical care setting or hospice prefered.
Required:
Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
