Overseeing care coordination Full Job Description Come join our team!! Wages commensurate with experience. As the MDS Coordinator, you will lead all aspects of the resident assessments, care planning, completion of the MDS and RAPS.The MDS Coordinator compiles information and data from health records and personal assessments for use in the evaluation of quality of care and utilization review. You will determine the most appropriate time for admission, quarterly, annual and significant change assessments. As the lead in this area, the MDS Coordinator will ensure all team members are aware of and complete scheduled and unscheduled assessments.

Job Requirements:

• Actively licensed as a Registered Nurse in State of Iowa
• Computer proficient and previous MDS experience is preferred
• Certified Resident Assessment Coordinator or willing to obtain certification within six (6) months of employment.
