REGISTERED NURSE - CARDIAC REHABILITATION

CRHC is looking for a PRN Registered Nurse to join our Cardiac Rehab team. Hours and days will vary between 6:30 am - 5:30 PM, M-F.

PRIMARY FUNCTION

Responsible for Advancing Exceptional Care by prioritizing Safety and Quality in every aspect of patient care. QUALIFICATIONS

Education and/or Experience Graduate from an approved school/college of nursing One-year RN experience required Familiar with telemetry monitoring and dysrhythmia, preferable experience in ER, ICU, medical surgical unit, and/or post-operative PACU, etc. Settings where telemetry is to be monitored. WORK SCHEDULE & SHIFT REQUIREMENTS

The facility operates 24/7; however, work schedules may vary based on departmental needs to ensure the highest standard of patient care and operational efficiency. Flexibility will be expected. Weekend and Holiday rotation as needed. Eligible for shift differentials where applicable Position is classified as non-exempt and eligible for overtime in accordance with labor laws.

Utilizes the nursing process, for the performance of patient care, to access, plan, intervene, and evaluate patient care to achieve prioritized patient outcomes. Preforms complete admission assessment, including full body system analysis and home med reconciliation process for outpatients. Preforms patient/family teaching for assessed knowledge deficits. Evaluates patient progress toward desired outcomes prior to discharge. Demonstrates personal accountability in the delivery of patient care in a competent, professional and compassionate manner. Demonstrates knowledge of monitors, computers, glucometers, oximeters, and exercise equipment. Performs admission and assessment activities for each patient prior to being seen by the clinic provider according to the reason for the visit. Follows admissions and assessment guidelines check list (full set of vitals). Documents allergies and type of reaction upon admission. Conducts applicable age-appropriate assessments effectively. Reviews and updates medication list weekly. Preforms emergency treatment as required in accordance with RN scope of practice. Initiates or assists in patient care in an effective and responsible manner as each situation requires. Triages each emergency appropriately and involves other resources as appropriate. Follows emergency policies, procedures and protocols in an effective MANNER. Demonstrates the ability to remain calm in emergent and stressful situations. Preforms and maintains documentation. Documents accurate and ongoing assessments of patient status reflect nursing interventions, patient response, patient teaching and status of outcomes as discharge. Completes shift end check with another nurse prior to leaving duty if applicable. Communicates effectively. Communicates and collaborates effectively with other health team members regarding patient condition, nursing plan of care, patient satisfaction needs and recommendations for meeting identified outcomes. Follows approved report format (Huddle) when communicating patient information. Identifies and recognizes abnormal symptoms/changes in patient condition, established priorities, and takes appropriate action. Appropriately reports condition changes to medical provider. Promotes and ensures patient safety in performance of all responsibilities. Administers exercise prescription in a timely and safe manner according to physician orders and rehab policy. Demonstrates proficient technical/clinical skills operational knowledge of equipment on unit. Informs and involves Manager regarding patient care issues in a timely and appropriate manner. Utilizes proper body mechanics, transfer/lifting techniques and appropriate equipment to minimize fall risk to patient and injury to self. Quality Program Participation Actively participates in CRHC's Quality Program, committees, performance improvement initiatives, and operational support activities. Demonstrates a commitment to Quality Improvement (QI) practices. Regularly attends Department huddles. Completes tasks as outlined on the Department Huddle Board. Other Responsibilities Performs additional duties as assigned to support the strategic initiatives of CRHC.
