Job Description

Summary The Iowa City Veterans Affairs Health Care System (ICVAHCS) is seeking a full-time Advanced Practice Nurse to join the Mental Health Service Line at the VA Clinic in Decorah

  • seeking to create a personalized health plan for each Veteran This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.
Major duties and responsibilities of the position: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice Develops and implements an individualized plan to achieve optimal population health and disease management Demonstrates leadership and works collaboratively with the interdisciplinary team Orders and interprets labs
