RN Case Manager, Educator & Infection Preventionist 609 SE Kent Street, Greenfield, IA 50849 Full-time Full-time Registered Nurse (RN) - Case Manager, Educator, & Infection Preventionist Adair County Health System (ACHS) is seeking a compassionate, skilled Registered Nurse (RN) to serve in a combined role as Case Manager, Clinical Educator, and Infection Preventionist . ESSENTIAL FUNCTIONS 1.
Case Management a. Coordinate and facilitate patient care across the continuum for both inpatient and outpatient populations b. Assess patients for discharge needs, barriers to care, and appropriate level of services c. Develop, implement, and adjust individualized care and discharge plans in collaboration with providers, nursing, patients, and families d. Coordinate timely and appropriate discharges, including referrals to post-acute services (home health, nursing home, DME, etc) e. Serve as a liaison between the patient, family, healthcare team, and external providers to ensure continuity of care f. Assist with transitions of care, including follow-up appointments, medication access, and community resources g. Identify high-risk or complex patients and provide focused coordination to improve outcomes and reduce avoidable readmissions or repeat ER patients h. Work with nursing and providers on education, resources, and discharge instructions/plans for patients and families i. Lead interdisciplinary care rounds and planning j. Maintain knowledge of community and local resources and establish partnerships with external partners to support patient needs k. Perform utilization review to assess medical necessity and determine appropriate level of care (inpatient, observation, outpatient), ensuring alignment with regulatory guidelines and payer requirements. l. Complete prior authorizations for observation, skilled, and acute patients in accordance with payer requirements, ensuring timely approval and preventing delays in care. m. Assist patients with advance directives, including providing education, facilitating completion, and ensuring documentation is included in the medical record. n. Promote and facilitate appropriate skilled nursing facility admissions and acute care transfers from referring facilities through collaboration, relationship building, and care coordination to support patient access and hospital volume. o. Develop, implement, and maintain policies and procedures for case management, discharge planning, and utilization review in accordance with regulatory and payer requirements. 2. Infection Prevention a. Collaborate with organizational leadership and committees (including CIAPIC, QPSIP, P&T, and Antimicrobial Stewardship) to support infection prevention, antimicrobial stewardship, and regulatory compliance activities. b. Monitor, analyze, and report quality and infection prevention metrics, including quality scorecard data, monthly NHSN reporting, and hospital-acquired infection surveillance. c. Conduct chart audits, concurrent and retrospective surveillance, and compliance monitoring to evaluate infection prevention practices, isolation adherence, and clinical documentation accuracy. d. Ensure completion of required infection prevention activities, including hand hygiene audits, ICRA management, TB risk assessments, and employee absentee tracking and follow-up. e. Develop, review, and maintain infection prevention policies and procedures in alignment with regulatory requirements and evidence-based practices. f. Coordinate and oversee the annual Infection Prevention Program activities, including the Infection Prevention Plan, risk assessment, and program evaluation. g. Provide infection prevention education and mentoring, including new employee orientation, ongoing competency training, biannual skills education, and on-demand staff education. h. Support staff development and compliance through coordination of educational programs, training records, and certification tracking. i. Participate in monthly CIAPIC meetings and other required interdisciplinary committees to support infection prevention initiatives and organizational goals. j. Collaborate with Quality Manager and Employee Health nurse 3. Educator a. Coordinate and deliver new employee clinical orientation, including onboarding support and competency validation. b. Provide ongoing monthly education sessions for inpatient units and other departments as needed based on identified learning needs. c. Work collaboratively with all department managers, including the Quality Manager, to identify and address targeted education needs across the organization. d. Support new clinical hires with skills validation, check-offs, and competency verification. e. Plan, coordinate, and facilitate bi-annual skills competency days for clinical staff. f. Coordinate simulation-based education opportunities, including mobile simulation ("sim truck") and other hands-on learning experiences. g. Assist departments with development and completion of orientation checklists and competency tools. h. Ensure staff education aligns with regulatory, accreditation, and compliance requirements. i. Identify educational needs across departments and develop or coordinate targeted learning interventions. j. Maintain documentation of education, competencies, and training compliance records. k. Perform other duties as assigned. Licensed in the State of Iowa as a Registered Nurse, Bachelor's degree preferred but not required 2 years of hospital nursing experience required Case management experience preferred but not required BLS required upon hire or before completion of orientation ACLS, PALS, TNCC, required within 6 months of hire Adair County Health System (ACHS) is a network of health care facilities and services focused on providing exceptional care to individuals in Adair County and the surrounding area.
