Job Description

SCHEDULE
Full-time; 36 hours per week; 4 days per week; On-call time every 3rd weekend and 1-2 days per week The Hospice RN utilizes the nursing process to provide and direct quality patient care. The Hospice RN provides leadership when working with team members in maintaining standards of professional nursing practice. The Hospice RN participates with the interdisciplinary team to develop the plan of care and supports Home Health and Hospice philosophies in caring for clients. The Hospice RN promotes health and wellness through education, clinical practice and home care for the ill and disabled. Current Iowa license as Registered Professional Nurse. Must possess valid Iowa driver's license and maintain auto liability insurance. Certification in Basic Life Support is required. Experience in acute care setting, home health or hospice preferred. Utilizes and builds upon knowledge acquired through formal education. Takes responsibility for pursuit of continuing education. Understands and utilizes the nursing process. Takes responsibility for knowledge of death and dying process, grief, bereavement and nursing management of a hospice/home health patient. Completes assessments/reassessments, pertinent documentation, plan of care and evaluations on time. Makes visits according to orders. Identifies, initiates and provides ongoing education to client, family, caregiver, significant other, etc. on issues identified during assessment/reassessment process. Is competent within scope of practice and provides age-appropriate care (infant, pediatric, adult and geriatric). Listens attentively to others in order to fully understand others' needs. Displays kindness and offers help. Listens thoughtfully and empathetically and responds appropriately.
