Homes Nursing Care Coordinator ChildServe - 4.1 Johnston, IA Job Details Full-time 18 hours ago Benefits Flexible schedule Qualifications Working with individuals with developmental disabilities Working with individuals with neurodevelopmental disorders RN License Writing skills Working with pediatric populations English Working with children Pediatrics Working with children with disabilities Medical terminology Full Job Description The Homes Nursing Care Coordinator is directly responsible for ensuring the clinical needs of each child is assessed, planned, and evaluated. You will facilitate a multidisciplinary team approach to clinical care using appropriate resources to develop and implement an individualized teaching and discharge plan with the children and their families. When You'll Work On call Monday- Friday 6am-4pm with flexible working schedule. Back up on call every 5th weekend and 1 holiday. What You'll Do Assess, plan, and evaluate the child's clinical care Ensure care plans are implemented correctly by other staff; provide training as needed. Facilitate multidisciplinary care team meetings for each child at least annually and as needed per regulatory requirements. Achieve CHOROUS results for their scope (i.e Care Plan goal achievement, care plans within 30 days, care plan reviews annually). Provide on-site nursing and attend health-related appointments with children. Maintain accurate, completed, and timely records and provide feedback to leadership on how the documentation system can be improved. Communicate effectively, respectfully, and professionally with children, physicians, families, school personnel, Child Serve staff, and others to assure assigned child's care needs are assessed, communicated, and met. Provide oversight of the care delivered by Licensed Practical Nurses and paraprofessionals. Provide training to children, families, and staff as needed to help advance the skills and knowledge of all people associated with a child's care delivery. Serve as a preceptor/mentor to new care givers as assigned. Coach and support peers in ways that improve care and documentation. What You'll Need Graduate of an accredited School of Nursing and licensed to practice as an RN in the State of Iowa. RN experience in the field of developmental disabilities, pediatrics and skilled patients. Documentation of current CPR/BLS certification. Experience as Case Manager, Care Manager, or care coordinator preferred. Knowledge of clinical terminology and modalities of care delivery systems; good communication and organizational skills; skilled in the use of computers; commitment in providing premiere nursing services. Ability to communicate effectively in reading, writing, and speaking the English language Who We Are At Child Serve , no position is ever "just a job."
