Develop, implement, and update individualized care plans based on physician orders and patient needs. Identify therapeutic, preventive, and rehabilitative interventions that support positive patient outcomes. Direct Patient Care Provide skilled nursing care in accordance with physician orders and the State Nurse Practice Act. Administer medications, treatments, and clinical interventions as prescribed. Monitor patient conditions and make appropriate recommendations for care plan adjustments. Educate patients and caregivers on diagnoses, medications, treatments, and disease management. Care Coordination Coordinate care across multiple disciplines to ensure seamless service delivery. Collaborate with physicians, therapists, social workers, home health aides, and community providers. Advocate for patient needs and help connect families with available resources. Documentation & Communication Maintain accurate, timely, and compliant clinical documentation. Communicate patient status changes promptly to physicians and obtain necessary orders. Provide regular updates to the care team to ensure continuity of care. Patient & Family Education Empower patients and families through education and ongoing support. Encourage caregiver involvement in the care planning process. Promote independence, safety, and improved quality of life. Supervision & Leadership Instruct, supervise, and evaluate home health aides according to agency standards. Qualifications:
Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting. Current RN licensure and CPR certification. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
