Job Description

Nursing degree, active Nurse Practitioner (NP) license and minimum 1+ years Home Health Nurse Practitioner (NP) experience required. Applicants who do not meet these qualifications will not be considered. Deliver meaningful care while enjoying maximum flexibility as a Nurse Practitioner supporting home health risk assessments. This per diem opportunity puts your expertise to work in Bingham County, including Blackfoot, Shelley, and Aberdeen, by visiting Medicare Advantage plan members in their homes. You will have full control over your schedule, choosing shifts that suit your availability anywhere between Monday and Sunday, with a minimum weekly or bi-weekly hourly commitment according to your preferences.

Key Responsibilities:
Complete annual home health risk assessments on-site for Medicare Advantage plan members Review and evaluate past/current medical history, documented conditions, and medications Deliver clear education to members about their health and prescribed medications Detect gaps in care and make recommendations for appropriate follow-up No treatment plan creation or medication prescribing required
Qualifications:
Active Nurse Practitioner license in Idaho Board Certification through AANP or
ANCC MSN
degree earned from an accredited nursing school Current CPR certification (BLS) Six months' experience as a Nurse Practitioner preferred Experience the reward of making a direct impact on patient care in your community while benefiting from a highly flexible per diem opportunity.
