• Full-time A well-established hospice organization is seeking a full-time RN Case Manager to provide compassionate, patient-centered care in a community-based setting.

This role is ideal for a nurse who values meaningful work, interdisciplinary collaboration, and balanced caseloads. The RN Case Manager is responsible for coordinating and delivering hospice care services to assigned patients while supporting families through the end-of-life journey. What You'll Do:

Manage 10-14 patients, ensuring they receive appropriate care. Complete assessments, create care plans, and document care. Collaborate with doctors, social workers, and other team members. Provide support and education to families. Enjoy flexible scheduling and work-life balance (avoid burnout!).

We're Looking For:

Idaho RN License. 2+ years of experience (pref. hospice). Strong communication and teamwork skills. Valid driver's license & reliable transportation.
