ICD-10 CM
diagnosis codes to the residents' medical conditions Accountable for case management of Medicare Part A and insurance case management Expertly assess nursing home residents' needs, and coordinating personalized, resident-driven care based on those assessments Oversee the patient assessment process, setting assessment schedules and assuring that assessments are done in an accurate and timely manner Make Recommendations and transmit assessments to the Dept. of Health. Review the results to make policy and procedural recommendations to the management staff Coordinate patient care plan in accordance with regulatory requirements. Ensure that appropriate facility resources are made available to patients and that patient care is delivered effectively Create the schedule for all Medicare and Medicaid including: Start Medicare coverage for newly qualified patients and/or send out denial letters Keep updated on regulatory changes in Medicare coverage and help determine documents needed for Medicaid reimbursement.
