• 5:00 PM | 40 Hours/Week Guaranteed We are seeking an experienced and compassionate Registered Nurse (RN) for a Home Health assignment in Boise.

This opportunity is ideal for nurses with strong home health experience who are dedicated to providing exceptional patient-centered care in a home setting.

Responsibilities:

Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans Conduct comprehensive patient assessments and ongoing evaluations Develop, implement, and update patient care plans based on clinical findings and patient needs Complete accurate OASIS documentation and maintain timely electronic medical records Administer medications, wound care, treatments, and other nursing interventions as prescribed Educate patients and family members regarding medications, disease management, treatment plans, and home care instructions Coordinate care with physicians, therapists, case managers, and interdisciplinary healthcare teams Monitor patient progress and report changes in condition to appropriate providers Ensure compliance with home health regulations, infection control standards, and agency policies Maintain productivity standards and manage daily scheduling efficiently

Requirements:

Active Registered Nurse (RN) license required Current BLS/CPR certification required Minimum 1 year recent Home Health RN experience required OASIS documentation and billing experience required Experience with Homecare Homebase software required Strong clinical assessment and care coordination skills required Excellent communication, time management, and documentation abilities required Ability to work independently and manage patient caseload effectively required
