What You Can Expect:
The Home Health RN Care Coordinator would work with physicians and multidisciplinary team members to develop a plan of care for assigned patients. You will also plan, implement, and document nursing care based on patient's needs. You will also make sure the patient is progressing toward desired outcomes by monitoring care through assessment and/or patient records. St. Luke's Home Healthand its interdisciplinary team values integrity, compassion, accountability, respect and excellence with all the patients receiving home health. The team works collaboratively to provide quality health care to patients and families in the community. Reliable personal transportation is required during work hours with current auto registration and auto insurance.
Hours will be between:
Monday - Friday (8:30am - 5:00pm) with rotating/variable weekends including rotating/variable weekday and weekend daytime/evening call up to 8:00 pm.
Qualifications:
Nursing Degree Experience:
2 years of RN experience
Licenses/Certifications:
Current RN License in the State of Practice Valid Driver's License and Auto Insurance if work related travel is required for position Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
