Job Description

Elite is seeking compassionate and dedicated nurse practitioners to join our team in providing exceptional home health and hospice care. What You'll Do:
Provide palliative care through evaluation and assessment of health history, diagnosing medical conditions. Develop individualized plans of care. Complete face to face assessments and prescribe medications or treatments. Collaborate with the interdisciplinary team.
What You Bring:
Active NP certification (state-licensed). Valid driver's license, reliable transportation, and current auto insurance. Strong communication skills and a team-oriented mindset Ability to work independently in home settings Commitment to patient-focused care

Wage:
$55 - 65 per hour
