St. Luke's Health System in McCall, ID is seeking a Home Health Case Manager to join our Home Health team. The Case Manager will occasionally be required to provide support to Hospice patients in the McCall area. Position Overview As a Home Health Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within the McCall and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan. Monday to Friday, 8:30 AM - 5:00 PM, with rotating evening on-call and weekend shifts, Opportunity to work primarily on weekends and cover occasional weekday shifts as well.Patient Population :
Home Health and Hospice patients within the McCall service areaManageable Caseloads :
Daily visit requirements are below the state and national averageCompensation :
Paid hourly which includes drive time and mileage reimbursement for traveling within McCall and it's surrounding regionsAdditional Pay :
On-call pay and applicable differential payBonuses :

ASN or BSN inNursing Experience :
2 years of RN experience; Home Health or Acute Care experience preferredLicenses/Certifications :
Current RN license in the state of Idaho; Basic Life Support (BLS) certificationOther Requirements :
