Palliative Care Nurse Practitioner (New Grads - Only with Previous Hospice/PC Experience) Arlington Heights, IL 60005 Part-time, Full-time Part-time, Full-time Job Summary We are seeking a compassionate Palliative Care Nurse Practitioner to join our multidisciplinary healthcare team. This role involves providing comprehensive symptom management, emotional support, and quality-of-life care to patients with serious illnesses. The ideal candidate will have extensive experience in hospice and palliative medicine and a strong background in managing complex medical conditions in a nursing facility setting. Knowledge of electronic health record systems such as PointClickCare is essential to ensure accurate documentation and seamless patient care coordination. New Grad NPs welcome to apply only with previous hospice/palliative care experience. Duties Conduct thorough patient assessments, physical examinations, and review of medical history using advanced clinical evaluation techniques. Develop and implement individualized palliative care plans focusing on symptom control such as pain management. Coordinate with multidisciplinary teams for discharge planning, case management, nutrition counseling, and family education. Manage complex cases involving hospice & palliative medicine. Utilize EMR/EHR systems for accurate medical documentation and coding (ICD-10/CPT). Provide updates to family on every visit. Ensure infection control protocols are strictly followed during all procedures such as sterile processing or aseptic technique applications. Engage in patient education regarding disease processes, medication administration, nutrition, and health coaching strategies. Skills Proven experience in hospice & palliative medicine. Expertise in managing complex medical conditions. Proficiency with EMR/EHR systems, including PointClickCare, familiarity with medical coding (ICD-10) and CPT coding for billing purposes. Strong knowledge of physiology and anatomy relevant to internal medicine, geriatrics, behavioral health issues, and infectious disease protocols. Ability to perform diagnostic evaluations using laboratory experience or medical imaging techniques. Excellent communication skills for patient education and family support; adept at working with diverse populations. Knowledge of HIPAA regulations to ensure patient confidentiality; experience working within NCQA standards is preferred.
