Position Summary High Point Residence Fox Valley; a supportive living community is seeking a compassionate and dedicated RN or LPN to join our team in a supportive living community setting. This full-time role plays a key role in delivering high-quality, person-centered care under the direction of the Wellness Director. This is an excellent opportunity for a nurse who enjoys working closely with residents and being part of a collaborative care team. Key Responsibilities Provide routine daily nursing care and clinical services in accordance with individualized care plans Perform health assessments, monitor vital signs, and report changes in condition Administer medications accurately and maintain proper documentation Collaborate with CNAs and team members to support resident well-being Provide clinical leadership and guidance to direct care staff Promote a safe, positive, and resident-focused environment Perform additional duties as assigned by the Executive Director or Wellness Director Qualifications Current Illinois RN or LPN license in good standing Strong clinical assessment, leadership, and communication skills Proficiency in medication administration and care documentation Valid CPR certification required Requirements Ability to lift up to 25 lbs. and assist residents as needed Experience in supportive living, assisted living, senior living, or long-term care preferred Equal Opportunity Statement High Point Residence Peoria is proud to be an Equal Opportunity Employer.
