MDS Nurse

Meadowbrook Manor in Bolingbrook is seeking an experienced MDS ( RN ) to join our team. We want you to be able to focus on assessing our residents, evaluate and maintain MDS's for timely, accurate completion and submission. There are no floor nursing responsibilities with this role.

Responsibilities:

Assess residents in accurately complete the Minimum Data Set and Resident Assessment Protocol Implement and monitor individual care plans for residents based on MDS Guidelines Maintain a schedule of due dates for MDS and Care Plans including new admissions and those with significant changes. Submit the MDS to the appropriate agency. Strong written and verbal communication skills and critical thinking abilities Solid understanding of current state, federal and local standards, regulations and guidelines that govern our facility Ability to organize and prioritize.

Requirements:

Illinois RN License in good standing. 1 year Experience in LTC/SNF as an
