Technical Proficiency Full Job Description Job Summary:

The nurse navigator provides comprehensive coordination of care for patients and their families/caregivers from screening to detection to diagnosis and treatment.

Essential Functions and Duties:

Responsible for the planning, scheduling, and implementation of day-to-day clinical activities and procedures for mammography and CT lung screening programs Guides patients and families through the screening, enrollment, and scheduling process Assist patients with referrals to outside resources and care coordination as needed. Ensure programs meet ACR and MQSA guidelines Stays current with relevant and current literature and practice standards Participates in the lung cancer network meeting and other relevant committees deemed necessary Organizes and/or participates in program outreach Performs other duties as assigned Provides educational programs and in-services to hospital coworkers and community to increase awareness

Position Qualifications:

Active IL nursing license from an accredited nursing program Requires excellent leadership, organizational, written, and verbal communication and excellent interpersonal skills Ability to take initiative and work independently with minimum supervision Ability to implement professional and community-based education programs Knowledge of computer systems and Microsoft applications Current BLS certification to be renewed as required

Experience:

1 year of nursing experience preferred 2
