We weigh Start of Care higher, include non-visit care management points each week, and additional points for high tech visits. Typical work week is Mon - Fri with opportunities to see patients on the weekends. Overview:

OASIS Start of Care and case management Assessing or re-evaluating the patient's condition and needs to determine plan of care Provide patient care according to patient's care plan and needs Teach the patient and/or family members about care and support skills Medication management and education for patient and/or family Communicate regularly with Clinical Care Coordinators, Interdisciplinary team, and overseeing physician Accurately and thoroughly document patient care within company policy Supervision of License Practical Nurse. Develop care plan and supervision of home health aid when applicable. Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life.

Qualifications:

Licensed as a Registered Nurse or LPN in the state of Illinois with good standings Valid Driver's License and reliable vehicle with current insurance Current CPR Certification Prior Nursing experience in a Home Health, Hospice, Rehab, or Hospital setting is required Experience with Electronic Medical Records (EMR) software in home health Comfortable with technology - a company tablet will be provided Strong communication and interpersonal skills Time management to balance patient care, travel, and documentation Great customer service
