Coordinator will play a pivotal role in ensuring the highest quality of care for our residents. This is a FullTime opportunity for a motivated and compassionate individual to join our team in East Moline, Illinois, United States . About the
Role:
Collaborate with interdisciplinary teams to develop and implement individualized care plans for residents Coordinate and facilitate the MDS process, ensuring accurate and timely completion of assessments and care plans Utilize clinical knowledge and expertise to identify and address resident needs, concerns, and goals Communicate effectively with residents, families, and healthcare professionals to ensure seamless care transitions and coordination
Responsibilities:
Conduct resident assessments and develop care plans in collaboration with the healthcare team Coordinate and facilitate MDS team meetings to ensure accurate and timely completion of assessments and care plans Monitor and document resident progress, communicating changes and concerns to the healthcare team Participate in quality improvement initiatives to enhance resident care and satisfaction Collaborate with staff to develop and implement resident-centered care strategies
Requirements:
Licensed Registered Nurse (RN) or Licensed Practical/Vocational Nurse (LPN/LVN) Experience in long-term care or rehabilitation setting Strong clinical knowledge and assessment skills Excellent communication and customer service skills Ability to work effectively in a fast-paced environment with multiple priorities
