Fairfield Senior Living & Rehabilitation is seeking a dedicated, detail-oriented MDS Nurse Manager to join our leadership team. This is an excellent opportunity for an experienced nurse who is passionate about accurate assessments, strong care planning, regulatory compliance, and improving resident outcomes. In this role, you will play a key part in our interdisciplinary team, ensuring clinical accuracy, timely MDS completion, and high-quality, person-centered care in a supportive environment. What You'll Do Oversee and complete MDS assessments in accordance with regulatory requirements Ensure accuracy of documentation to support resident care plans and reimbursement Collaborate closely with nursing, therapy, and administrative teams Monitor and update care plans based on resident condition changes Maintain compliance with Medicare/Medicaid guidelines and facility policies Participate in interdisciplinary meetings and care coordination Requirements Active RN or LPN license in good standing in the state of Illinois Prior experience in MDS coordination and care planning required Strong knowledge of Medicare/Medicaid regulations and reimbursement processes Excellent communication skills (written, verbal, and interpersonal) Highly organized, detail-oriented, and able to manage multiple priorities Ability to work Monday-Friday, 8:00 AM-4:30 PM Participation in on-call rotation as needed Strong ability to collaborate across all departments If you're looking for a leadership role where your expertise is valued and your work directly impacts resident quality of care, we'd love to connect with you.
