at Mercy Health Corporation (Mercyhealth) in Freeport, Illinois, United States Job Description Essential Duties and Responsibilities Assess patient data and perform interview, physical examination and begin discussing goals of care within prescribed guidelines and instructions of Physician. Maintain timely consult schedule. Provide patient and family education to assist patient and family to understand role of palliative care in current health situation. Order, interpret, and evaluate diagnostic tests to identify and assess patient clinical problems and health care needs if required. Record physical findings, and formulate plan and prognosis, based on patient condition. Coordinate care with Palliative Care Physician and other health professionals; preparing comprehensive patient care plan. Discuss health care plan and goals of care with Physician partner. Consult with partnering physician and refer to specialized health resources for treatment and evaluation as needed. Assists in developing palliative care protocols and processes.
