Schedule:
Full-Time/Day Shift:
Day Shift Monday-Friday 8am to 5pm with e/o weekend or call rotation.
Location:
Forest park Maywood Berwyn North Riverside Forest View Westchester Area with travel to LaGrange Hospital for meetings/Orientation etc. Implements and monitors patient care plans to ensure effective treatment. Monitors, records, and communicates patient condition as appropriate. Serves as the primary coordinator of all disciplines for well-coordinated patient care. Notes and carries out physician and nursing orders accurately and promptly. Conducts holistic, individualized assessments of each patient considering age, physiological, and psychological status. Administers medications and solutions as required. Documents all phases of patient status during care. Participates in the transfer of patient information to ensure continued quality care. Leads and mentors staff to ensure optimal performance and competency in care delivery. Maintains a safe and clean working environment by complying with policies, procedures, rules, and regulations. Develops and maintains positive working relationships within the practice and other departments. Other duties as assigned. Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
