MDS Coordinator and Restorative Director Registered Nurse (RN) South Elgin Living and Rehab LLC Joliet, IL Job Details Full-time 10 hours ago Qualifications RN License Computer literacy MDS Full Job Description This is a dual role and the candidate will be both the MDS Coordinator as well as the Restorative Coordinator. The candidate must be proficient in MDS and experienced in Restorative Therapies. This opportunity is open to a Registered Nurse (RN), licensed in the State of Illinois, who assumes the primary responsibilities for the care planning process and MDS completion within this facility and acts as the facilitator at the conference meeting. The Registered Nurse (RN )will also perform rehabilitation assessments upon all new admission and for existing patients throughout the center on quarterly basis and as needed and insures that each patient receives the Restorative Care, unique to them Duties and Responsibilities The Registered Nurse (RN) will be setting up and maintaining an organized system by which Care Plan/MDS review dates are determined and schedules. Notifying each department as to conference date, time, and meeting location as well as a list of specific residents to be reviewed. Maintaining a record of attendance at meetings by all staff members. Calling the meeting to order and acting as "timekeeper" throughout, ensuring that discussion is pertinent and the meeting ends on time. The Registered Nurse (RN) will be conducting the meeting efficiently and effectively, allowing each discipline the opportunity for input and questions and ensuring that the input is reflected with the MDS/Restorative plan by the Registered Nurse (RN) Eliciting necessary and appropriate information from staff members to promote the development of problems/need statements, goal and approaches. Responsibility for notifying family to invite them for care plan meeting. Making sure that departments are aware of changes in resident's conference. Relating information to or from the conference in the occasional absence of staff members. Ensure that all changes to MDS and Restorative Plans are done in a timely manner. Transmitted MDS Assessment within guideline outlined by Medicare and Medicaid. Making the Administrator aware of absence of staff members from conference. Submit copies of each transmittal report to the Administrator/DON. Discuss changes to MDS with each discipline as it relates to their area prior to changing codes. Perform other duties as assigned by the Administrator or DON. Performs or oversees functional evaluations on admission, quarterly and/or as clinically indicated. Consults with nurses in making recommendations for restorative plan of care. Works with nurses in developing restorative plan of care. Oversees that all restorative nursing measures are reflected on the care plan as an approach to the focus or need for which that are being completed. Works closely with the therapy department. Sees that specific recommendations, when discharged from therapy are carried through The Registerd Nurse (RN) reviews weekly documentation completed for residents receiving restorative services. Ensures that all proper ADL documentation has been completed for patients Oversees incontinent management programs Attends care planning conferences and makes recommendations. Reports progress of restorative nursing measures. Attends Interdisciplinary/QAPI meetings as indicated to provide feedback and identify any residents for evaluation of restorative service needs. Coordinates with Human Resources and Director of Nursing to orient new staff relative to roles, responsibilities, principles and practices of restorative nursing. Coordinates with Director of Nursing and Therapy to provide ongoing education/evaluation of direct care staff relative to roles, responsibilities, principles and practices of restorative nursing. Actively participates in physical restraint reduction and fall management programs. Sets up documentation flow sheets for daily documentation of restorative nursing measures for each resident on the program in coordination with medical records. Performs or supervises the performance of restorative nursing staff Serves as a liaison with all therapists: physical, occupational, and speech Participates in patient care planning and CMI meetings Working Conditions and Qualifications Be involved with residents, personnel, visitors, government agencies and personnel, etc. Be subject to hostile and emotionally upset residents, family members, etc. Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the department. Sit, stand, stoop, bend, lift and walk intermittently during the workday. Lift at least 25 lbs.

Transfer and turn residents. Qualifications The candidate must be a Registered Nurse (RN) in the State of Illinois Long term care experience. Familiarity with the function of each discipline involved in resident care and care planning. Effective communication and leadership skills. Strong sense of organization. Computer proficient.
