What You'll Do Registered Nurse -

Overview:

OASIS Start of Care, Recertification and Resumption and case management Assessing or re-evaluating the patient's condition and needs to determine homebound status and plan of care Provide patient care according to patient's care plan and needs Teach the patient and/or family members about care and support skills Medication management and education for patient and/or family Communicate regularly with Clinical Care Coordinators, Interdisciplinary team, and overseeing physician Accurately and thoroughly document patient care within company policy Supervision of License Practical Nurse and CNAs. Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life.

Qualifications:

Active Illinois RN license in good standing Valid driver's license, reliable vehicle, and auto insurance Current CPR Certification Prior experience in home health, hospice, rehab, or hospital setting is required Familiarity with Electronic Medical Records (EMR) software Comfortable with technology - a company tablet will be provided Strong communication and interpersonal skills Time management to balance patient care, travel, and documentation Great customer service !!
