25-32 points per week (approximately 3-5 patient visits daily) Field-based with daily travel to patient homes (mileage reimbursed) Key Responsibilities Conduct comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management Evaluate and monitor patient progress, communicating changes promptly to the care team and physicians Educate patients and families on therapies, medications, and preventive care strategies Maintain accurate documentation using Home Care Home Base (HCHB) electronic charting Collaborate with interdisciplinary healthcare professionals to ensure seamless patient care coordination Comply with all regulatory and agency standards regarding patient care and documentation Qualifications & Experience Active, in-good-standing RN license for the state of practice Minimum 1 year recent home health nursing experience with proficiency in fieldwork Experience using Home Care Home Base (HCHB) and OASIS assessment tool required Strong clinical assessment, critical thinking, and independent work skills Reliable transportation with valid driver's license and auto insurance Excellent communication, organization, and time management abilities BSN and knowledge of Medicare/home health compliance preferred Benefits Weekly pay Medical, dental, and vision insurance options 401(k) retirement plan options Mileage reimbursement for travel within assigned territory 24/7 support from the staffing team Potential for contract extensions and renewals This opportunity is ideal for an autonomous, experienced home health RN committed to delivering patient-centered care across a flexible field-based schedule. to join this rewarding role and impact patients' lives in their community.
