RN Case Manager, Home Health I $5,000 Bonus I Full-Time

Compensation:

$80,000 - $90,000+ based on experience, plus $5,000 bonus package. Aim Home Health has proudly served patients throughout the Chicagoland area for 15+ years. We are seeking an experienced RN Home Health Case Manager to join our team in a full-time, in-office clinical leadership role focused on patient care coordination, clinical oversight, and Medicare home health case management. •This role is ideal for an experienced nurse seeking a calmer, more structured, and supportive environment outside of the fast-paced hospital setting or large corporate healthcare systems. Primary Responsibilities:

Manage assigned patient episodes from Start of Care through discharge Review and maintain plans of care, physician orders, and visit frequencies Review OASIS documentation for accuracy, completeness, and compliance Coordinate care with RNs, LPNs, PT/OT/ST, MSWs, and HHAs Communicate with physicians regarding orders, recertifications, and changes in condition Monitor patient progress, goals, labs, skilled need, and discharge planning Ensure compliance with Medicare home health regulations and agency standards Participate in case conferences and quality improvement initiatives Support coordination between field clinicians and office operations

Qualifications:

Active Illinois RN license in good standing Minimum 2 years nursing experience required Home health case management experience strongly preferred Strong understanding of Medicare home health regulations and OASIS documentation Excellent communication, organizational, and clinical judgment skills Ability to manage multiple patient cases effectively and independently EMR/documentation proficiency required
