We are seeking experienced field RN's to join our healthcare team as a part-time nurse. You'll enjoy the flexibility and autonomy of creating your own weekly schedule along with highly competitive pay as well as a generous signing bonus.

Days

• Monday through Friday As a home health care RN with Northwest Home Health and Rehab, your visits are tailored to your geographical location and schedule. Home Health RN Responsibilities
• Function as a case manager in charge of caseload
• Complete initial Start of Care OASIS visit and regularly re-evaluate the patient's nursing needs
• Collaborate with physicians and the health care team in determining the care plan and needs of the patient
• Educate patients, caregivers and family members
• Work closely and collaboratively with all members of the health care team
• Inform the Clinical Manager of patient progress and clinical concerns
• Record patient medical information in our electronic medical record system, Axxess
• Inform the physician of changes in the patient's condition or treatment plans Not required to come in to the office. Home Health RN Qualifications
• Registered Nurse licensed in the state of Illinois
• Current driver's license, auto insurance and CPR
• Nursing experience in home health
• Effective written and verbal communication skills
• Strong desire to help others
