Position Overview We are seeking a compassionate and experienced Hospice Registered Nurse Case Manager (RN) to join our dynamic team. In this vital role, you will coordinate and deliver comprehensive hospice care to patients in their homes and outpatient settings throughout the Naperville and Aurora areas. Your clinical expertise and commitment to patient-centered care will help ensure comfort, dignity, and quality support for patients and their families during end-of-life care. This role offers a meaningful opportunity to make a lasting impact through hands-on case management and interdisciplinary collaboration. Responsibilities Conduct thorough patient assessments to determine eligibility and develop individualized care plans aligned with hospice standards. Manage a caseload of hospice patients, providing ongoing clinical oversight and ensuring quality of care. Coordinate interdisciplinary team activities including physicians, social workers, chaplains, and therapists to deliver holistic patient support. Monitor patient progress through regular vital signs checks, symptom management, and diagnostic evaluations. Educate patients and families on disease processes, medication management, discharge planning, and advanced directives. Document all patient interactions accurately in medical records while maintaining compliance with HIPAA regulations. Facilitate intake procedures for new patients and oversee post-acute care transitions across inpatient, outpatient, and clinic settings. Experience Hospice 1

$90,000.00 - $100,000.00 per year
