Responsibilities:

Perform skilled nursing visits in patients' homes Complete Start of Care (SOC), Recertification, ROC, Discharge, and routine nursing visits Assess patient condition and develop individualized care plans Educate patients and families regarding medications, treatments, and disease management Collaborate with physicians, therapists, and interdisciplinary team members Complete accurate and timely documentation

Qualifications:

Active Illinois RN License Minimum of 2 years of Med Surg experience preferred Home health experience strongly preferred Excellent communication and organizational skills Reliable transportation and valid driver's license Current BLS certification required Benefits for

$94,000.00 - $130,000.00 per year
