Job Description

Overview Willing to visit the Greater Chicago Areas, Northside and West of Chicago Join our dynamic healthcare team as a Visiting Registered Nurse (RN), where your expertise will make a meaningful difference in homebound patients. As a vital link in delivering comprehensive nursing services, you will provide skilled, compassionate care directly to patients in their homes. This role offers an exciting opportunity to utilize your clinical skills, including advanced procedures and specialized treatments, while working independently and collaboratively with multidisciplinary teams. Your dedication will support improved health outcomes and enhance quality of life for homebound patients. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to determine individualized care plans. Administer medications via injections, IV infusions, and other routes while adhering to aseptic techniques and safety protocols. Perform complex procedures such as catheterization, suturing, wound care, ostomy management, and tube feeding with precision and professionalism. Manage acute pain and provide specialized care for patients with chronic conditions like dementia, Alzheimer's disease, or developmental disabilities. Utilize a electronic health record (EHR) system such as Axxess to document patient visits accurately and efficiently. Coordinate discharge planning, case management, and collaboration with physicians for seamless continuity of care. Support emergency situations in home settings. Educate patients and families on health coaching, medication administration, infection control practices, and disease management strategies. Qualifications Valid RN license with current state registration; additional certifications such as Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS). Proven experience in diverse clinical environments, especially the hospital setting. Strong skills in medical documentation using EMR/EHR systems; familiarity with ICD-10 coding is a plus. Demonstrated proficiency in performing advanced procedures such as IV insertion, phlebotomy, laboratory specimen collection/processing, and sterile technique. Knowledge of physiology and anatomy to support accurate patient assessment and diagnostic evaluation. Experience working with individuals with disabilities or developmental challenges; familiarity with memory care or palliative medicine is advantageous. Critical thinking skills to manage acute pain cases, trauma medicine scenarios, and infectious disease care protocols. Ability to work independently in home or community settings while maintaining excellent communication within multidisciplinary teams.
