As a Home Visiting Nurse, you will be empowered to make a meaningful difference in patients' lives while working in a flexible and rewarding environment. Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to develop tailored care plans. Provide skilled nursing interventions such as wound care, medication management, and infection control following best practices and clinical guidelines. Educate patients and their families on disease management, medication usage, lifestyle modifications, and post-discharge care instructions. Coordinate with multidisciplinary teams to facilitate discharge planning, case management, and utilization management for optimal patient outcomes. Maintain accurate and detailed medical records in compliance with HIPAA regulations and organizational policies. Monitor patient progress through ongoing evaluation and adjust care plans accordingly to meet evolving needs. Ensure safety protocols are followed during home visits, including infection prevention measures and emergency response procedures. Qualifications Valid nursing license (RN) with current registration in the state of practice. Critical care experience preferred, with background in hospital medicine or emergency medicine environments such as ICU or Level I Trauma Centers. Experience in post-acute care settings including outpatient clinics or home health services is highly desirable. Knowledge of physiology, medical terminology, and diagnostic evaluation techniques essential for accurate patient assessment. Strong skills in patient service, discharge planning, case management, and utilization management required. Familiarity with infectious disease care, behavioral health considerations, geriatrics, pediatrics (PICU experience a plus), and hospital experience enhances effectiveness in this role. Ability to work independently while maintaining excellent communication skills; proficiency in medical records documentation is necessary.
