" Touching lives invigorates, inspires and drives us to do our very best, and we continuously look for ways to make each life better with the utmost care and professionalism Essential Duties and Responsibilities Assess patient data and performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician Maintain timely consult schedule. Provide patient and family education to assist patient and family to understand role of palliative care in current health situation. Order, interpret, and evaluate diagnostic tests to identify and assess patient clinical problems and health care needs if required. Record physical findings, and formulate plan and prognosis, based on patient condition. Coordinates care with Physician and other health professionals to prepare comprehensive patient care plan. Discusses health care plan and goals of individual patients for periodic review and evaluation by Physician. Refers patients to Physician for consultation or to specialized health resources for treatment. Assists in development of protocols. Education and Experience Graduate of an accredited nursing program. Graduate of an accredited masters of nursing program. Meets the requirements determined by the State of Wisconsin or Illinois to perform duties defined as those of a Nurse Practitioner. Certification and Licensure Hold a valid
APRN & RN
license in state of practice. Hold a valid DEA license in state of practice. CPR or BLS certification or other certifications as required
