If you're an RN who wants your work to mean something beyond a chart, this is your role. What You'll Do As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most.
Your day-to-day includes:

Requirements:
What You Bring Active RN license in the state of employment Graduate of an accredited school of Nursing. One (1) + year Hospice, home health or related organization experience preferred. Compassion for patients and families navigating end-of-life care Strong clinical judgment and the ability to work independently in the field Excellent communication skills across care teams and families
