We're looking for a compassionate, skilled RN to join our team and serve patients where it matters most: in the comfort of their own homes. What You'll Do You won't be just another face on a rotating floor schedule. Here, you'll own your patients' care from assessment to outcome — building real relationships and making decisions that matter. Conduct thorough assessments of patients' physical, emotional, and psychosocial needs to build a complete picture of their health and wellbeing Develop and lead individualized care plans alongside a collaborative interdisciplinary team — physicians, therapists, social workers, and aides all working toward the same goal Deliver hands-on skilled nursing care including wound care, IV therapy, medication management, and symptom management (with a strong focus on pain and comfort for hospice patients) Educate and support patients and their families — equipping caregivers with the knowledge and confidence they need between visits Guide and supervise LPNs/LVNs, home health aides, and support staff Monitor patient progress closely and adjust care plans as conditions evolve Participate in on-call rotations, admissions, and urgent response as needed Maintain accurate, compliant clinical documentation that reflects the quality of care you deliver What We're Looking For You're a nurse who takes ownership. You're calm under pressure, thorough in your documentation, and genuinely invested in the people you serve.
Requirements:
Graduate of an accredited school of professional nursing Active RN licensure in the state of agency operation Minimum 1 year of RN experience Valid CPR certification (must be maintained throughout employment) Valid driver's license, reliable insured vehicle, and willingness to travel to patient locations Why This Role Stands Out Home health and hospice nursing isn't for everyone — but for those called to it, there's nothing more rewarding.
