Duties Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to determine care needs. Manage patient care plans by coordinating treatments, medication administration, and follow-up evaluations in outpatient and home settings. Provide acute care interventions for patients with complex conditions such as infectious diseases, trauma-related injuries, or post-acute recovery needs. Educate patients and their families on disease management, medication adherence, lifestyle modifications, and discharge instructions. Document all patient interactions accurately using electronic health records (EHR), ensuring compliance with HIPAA regulations and medical terminology standards. Collaborate with healthcare providers to develop personalized care strategies and facilitate seamless discharge planning. Assist in managing case loads by prioritizing patient visits, tracking progress, and adjusting care plans as necessary to optimize health outcomes. Requirements Knows how to do OASIS Strong background in hospital medicine, medical-surgical nursing, or outpatient clinic experience with a focus on acute or post-acute care. Knowledge of health regulation policies including HIPAA compliance and medical records management. Demonstrated ability to perform patient assessments accurately and manage complex cases involving geriatrics, pediatrics, behavioral health, or infectious diseases. Excellent communication skills for patient education, interdisciplinary collaboration, and documentation clarity. Valid nursing license with current registration; additional certifications in trauma care or emergency medicine are preferred. Ability to operate electronic health record systems efficiently and maintain meticulous medical documentation. Flexibility to work independently across various community settings while adhering to safety protocols and infection control standards.
