• Conduct patient assessments to determine healthcare needs and develop individualized care plans
• Coordinate and monitor patient care, ensuring timely and appropriate services are provided
• Collaborate with healthcare providers, including physicians, specialists, and ancillary staff, to ensure continuity of care
• Utilize medical terminology and knowledge of physiology to accurately document patient information and maintain medical records
• Provide patient education on disease management Experience
• Strong knowledge of medical terminology, patient assessment, and diagnostic evaluation
• Experience in case management or care coordination preferred
• Familiarity with primary care principles and utilization management processes
• Understanding of HIPAA regulations and ability to maintain patient confidentiality
